Insurance and Billing

Services may be provided in this health care facility by the facility as well as by other health care providers who may separately bill the patient and who may or may not participate with the same health insurers or health maintenance organizations as the facility.

Patients and prospective patients may request from this facility and other health care providers a more personalized estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider. To request a Patient Pricing Quote, please call (321) 369-9610.

Surgery Center of Melbourne participates with most major insurance carriers. We recommend that you check with your insurance provider to verify your coverage.

In addition to your bill from the center, you may receive a bill from other entities. Click here for a list of possible service providers that may charge you.

We are happy to assist you by filing all necessary insurance claims with your insurance provider for reimbursement. In order to do this, we will need your most recent insurance information on file. If your insurance plan requires a co-payment, payment is expected at the time of service. We accept cash, personal check, Visa, Discover and MasterCard. We offer the option to pay bills online. Pay our surgery center here. Pay for anesthesia services here.

We also accept Care Credit, a convenient, monthly payment program for yourself and your family specifically designed to pay for healthcare expenses not covered by insurance including co-payments, deductibles and elective treatment and care. Read more about Care Credit payment plans.

If full payment is not possible, we can assist you in formulating a payment plan. All balances not paid by insurance policies are the responsibility of the patient. If you need assistance with your bill, please do not hesitate to call us at (321) 725-5151.

The Cost of Your Procedure

Because you are responsible for costs for covered healthcare services until you reach your deductible, you should comparison shop if you elect to have a nonemergency procedure.

You can use tools like Healthcare Bluebook and the Medicare Procedure Price Lookup (for Medicare patients).  Many health insurance websites also provide information on where to find in-network services.
